Data entry application for Leade Health Inc. call intake center. Application includes login, search, call log, message center, data entry and scheduling functionality. Application includes dynamic question selection based on complex algorithms and instant data error checking. Design focus was on satisfying the varying needs of the call center personnel while maintaining a simple and flexible data entry system.

Role: Usability and information architecture consultation. Design interface graphics and layout. Code in XHTML, CSS and PHP, focusing on integration with PHP and database programming team.
